Faith Mountain Medical Release and Permission Form

All children's activities that are overnight or that take place outside our campus require a Medical Release and Behavior Expectation Form to be filled out by a parent  or guardian and then turned in to the Director and or person in charge of the activity. AWANA Parental Permission Consent

All children's participating in the AWANA Games require a Parental Permission and Medical Consent Form to be filled out by a parent  or guardian and then turned in to the Director and or person in charge of the activity. Registration Form
